Free Risk Assessment Template Singapore — Download WSH RA Template
Every Singapore workplace needs a proper Risk Assessment (RA). It’s not just a regulatory requirement from the Ministry of Manpower (MOM) — it’s the foundation of your entire safety management system. But creating one from scratch is time-consuming, especially if you’re not sure what format to use or what hazards to include.
That’s why we’ve created a free, MOM-compliant Risk Assessment template that you can download and customise for your business.
What is a Risk Assessment?
A Risk Assessment (RA) is a systematic process to identify workplace hazards, evaluate risks, and determine control measures to keep your workers safe. Here’s what you need to know:
- Required under Singapore’s Workplace Safety and Health (WSH) Act for all workplaces
- Must be reviewed at least once every 3 years, or whenever there’s a change in work processes
- Part of bizSAFE Level 2 and above certification requirements
- Must be conducted or endorsed by a competent person — such as a Workplace Safety and Health (WSH) officer, safety consultant, or Risk Management (RM) trained person
- Covers all types of hazards: physical, chemical, biological, ergonomic, and psychosocial

How to Complete Your Risk Assessment
Once you’ve downloaded the template, follow these seven steps to complete your Risk Assessment:
- List all work activities — Break down your operations into individual tasks. Include routine activities, non-routine tasks, and maintenance work.
- Identify hazards for each activity — What could go wrong? Think about falls from height, chemical exposure, electrical hazards, fire risks, manual handling injuries, noise exposure, and more.
- Assess existing controls — What safety measures are already in place? Document current engineering controls, administrative procedures, and PPE requirements.
- Evaluate the risk level — Use the likelihood × severity matrix in the template:
- Low (1–4): Acceptable risk. Monitor and review periodically.
- Medium (5–9): Additional controls needed. Review within 1 month.
- High (10–16): Immediate action required. Senior management attention.
- Extreme (20–25): Stop work immediately. Controls must be in place before work resumes.
- Determine additional controls — For Medium risk and above, identify what else needs to be done. Follow the Hierarchy of Controls: eliminate the hazard first, then substitute, then use engineering controls, then administrative controls, and PPE as a last resort.
- Assign responsibility — Who will implement each control measure, and by when? Set clear deadlines and follow up.
- Review and approve — The completed Risk Assessment must be endorsed by a competent person — a Workplace Safety and Health (WSH) officer, registered safety consultant, or RM-trained person.
Risk Assessment by Industry
Different industries face different hazards. Here’s a quick overview of the key risks by sector:
Construction
Working at height, lifting operations, excavation, hot work, confined space entry, falling objects, and structural collapse.
F&B / Food Manufacturing
Kitchen hazards, slips and falls on wet floors, chemical cleaning agents, fire risks, burns, and repetitive strain injuries.
Manufacturing
Machine guarding, noise exposure, chemical handling, electrical hazards, and manual handling of heavy materials.
Logistics & Warehousing
Forklift operations, manual handling, stacking and storage, loading and unloading, and vehicle movement in yards.
Office & Retail
Ergonomic hazards, electrical safety, fire evacuation, slip/trip/fall risks, and workplace stress.
Healthcare
Sharps and needlestick injuries, biological hazards, patient manual handling, workplace violence, and chemical exposure from cleaning agents.

When Do You Need a Professional Risk Assessment?
While the template is a great starting point, there are situations where you should engage a professional Workplace Safety and Health (WSH) consultant:
- bizSAFE Level 2 and above — a formal Risk Assessment is mandatory for certification
- Ministry of Manpower workplace inspection or Stop Work Order — you need a compliant RA urgently
- New workplace, new processes, or significant changes — any change in operations requires a fresh assessment
- After a workplace accident or near-miss — incident investigation should trigger an RA review
- ISO 45001 certification — requires a comprehensive occupational health and safety risk assessment
