A MAN has died after being crushed to death in a horror workplace accident at Ipswich, west of Brisbane. The man in his 60s was reportedly crushed by a scissor lift at a business in Carole Park on Saturday morning. Paramedics were called to the Cobalt Street warehouse around 1.20am, but it was too late.
